The winning card does not have to be the card responsible for most of a trick's penalty.
Hearts are led late in a hand. The legal plays are 3♥, 7♥, Q♠ and 5♦. The heart seven wins, collecting fifteen penalty points: two hearts and the spade queen.
The queen did not trump the hearts; Hearts has no trump suit. It raised the cost of the trick while the led suit determined its winner.
