Hearts: A Spade Queen Does Not Break Hearts
In the version where hearts must be broken before being led, a heart discard opens that permission. Q♠ is a penalty card but not a heart.

Only Q♠ has been discarded off suit so far. The next leader still has clubs and hearts. The queen's appearance does not by itself permit a heart lead.
Track the required event specifically. A penalty card appearing and a heart appearing are different events, even when both increase somebody's score.
